What you Deliver:

  • You work with clients, referred by supervisor, to enroll in program and identify eligible and available housing units for lease. Assists clients in gathering all documents necessary to complete a housing program enrollment. Accompanies clients to all housing appointments and serves as clients’ primary advocate during the housing search and selection process and prior to transfer to the housing case manager.

  • You assist clients to complete lease application, including assembling documentation needed for lease-up, and securing related assistance (application fee, etc.) as needed.

  • You maintain precise and accurate documentation of housing navigation services, including client files and entries into the program databases.

  • You attend Housing Assessor, Navigator, HMIS and other applicable trainings, as assigned, to effectively collaborate with partners and implement the navigation portion of the program effectively.

  • You coordinate with a variety of outside providers, as necessary, to meet client’s needs and avoid duplications of services. Provides community resource referrals for clients.

  • You perform other duties as assigned.
